Form submissions are delivered by encrypted email to the appropriate clinic office and retained only as long as needed to respond to you. Our website is served over HTTPS.

Your choices

You can request that we delete any contact information you’ve submitted by calling either office or emailing the address shown in the site footer.

Patient health information (HIPAA)

Information collected during care — including your medical history, evaluation findings, treatment notes, billing, and any communications about your care — is protected health information governed by our HIPAA Notice of Privacy Practices. A printed copy is provided at intake and is available on request.
